•The patient receives five or more visits per week from a hospice team who provides physical, emotional, and spiritual care through telehealth or in the safety of their own home, nursing home, or assisted living facility.
•Availability of trained clinicians around the clock who can dispatch a team member to the patient's residence if necessary.
•On-call care at home for up to 24 hours a day, seven days a week.
Melodia Care is also committed to providing a number of important community benefits, including:
•Veterans will have better access to health-care services.
•Volunteer opportunities that account for at least 5% of all direct-care hours will be available.
•Grants to local food, housing, and medical assistance programs, caregiver education and support, and scholarship opportunities are all available.
