Operator Failures 'Primary' Cause Of Titan Sub Disaster: US Coast Guard

Safety failures by the OceanGate company led to the deadly 2023 implosion of its Titan submersible, the US Coast Guard said in a final report published Tuesday.
The report identified the company's "failure to follow established engineering protocols for safety, testing, and maintenance" as the "primary causal factor" for the disaster, which killed five people on the dive to view the Titanic wreckage.
