Doctors At Aster Hospital Mankhool Rebuild 42-Year-Old Man's Face After Severe Accident Causes Complex Pan Facial Fractures

Having sustained severe facial fractures, following an accident, the patient arrived at the Emergency Room of the hospital. He had multiple deep cuts, heavy bleeding, broken bones across his face, double vision, difficulty breathing, and partial weakness in the facial nerves.

Doctors diagnosed severe pan facial trauma, a rare and highly complex condition involving fractures across the upper, middle, and lower regions of the face. Pan facial fractures account for only 4 to 10 percent of all facial fractures and are typically caused by high-intensity injuries.1 They are most prevalent among young adults, particularly in the 18–40 age group, making such cases both uncommon and extremely challenging to manage.2

In this patient's case, the injuries included multiple fractures of the jaw, cheekbones, nasal bones, and eye sockets, extensive loss of nasal cartilage and teeth, disruption of jaw alignment, and orbital injuries that posed a serious risk to vision. Due to the severity of the trauma, his nose was cut, his teeth were crushed, and his jaw was badly damaged, causing severe pain, heavy bleeding, and difficulty breathing.

The patient underwent a detailed surgical procedure called Open Reduction and Internal Fixation (ORIF) with multiple orbital reconstructions under general anesthesia. The surgery was led by Dr. Renju Prem, Lead Surgeon in Oral and Maxillofacial Surgery at Aster Hospital Mankhool, with anesthesia support from Dr. Abhishek J Narayan and Dr. Jawad Ebn Mohammed. The anesthesia team decided to opt for sub-mental intubation in view of extensive facial / nasal fractures. The team used multiple approaches to the maxillofacial skeleton starting from the mandible (“bottom to top approach”) to the cranial base. The external approaches were used carefully fix the broken bones, stabilize them with titanium plates and screws, rebuild the eye sockets, reposition the nerves, and restore the overall structure of the face. Damaged bone fragments and knocked-out teeth were removed, and the nasal structures were reconstructed to restore both function and appearance.

The surgery was followed by intensive monitoring in the ICU for three to four days, supported by cardiology and nephrology teams, after which the patient gradually transitioned to oral feeding and normal activity. After a total hospital stay of approximately two weeks, the patient was discharged in a stable, asymptomatic condition, having regained facial structure, jaw function, and overall stability.
