The patient, who had been experiencing poorly controlled chest pain despite medication, required a specialized intervention called Chronic Total Occlusion Percutaneous Coronary Intervention (CTO-PCI) to restore blood flow in a completely blocked heart artery. The Challenge

The case presented multiple obstacles that made it exceptionally difficult. The patient's Left Anterior Descending (LAD) artery - a critical vessel supplying blood to the heart - was completely blocked near a major branch point with what cardiologists call a "blunt stump," making wire navigation extremely challenging. Adding to the complexity, a previous attempt at another hospital had been unsuccessful.

"CTO-PCI requires specialized equipment including advanced guidewires, microcatheters, and sometimes intravascular ultrasound guidance," explained Dr. Nayak. "These are not routine cases, and success requires significant experience and technical skill, particularly when dealing with previously failed attempts."

The Solution

Dr. Nayak and his team employed an antegrade wire escalation technique, progressively using increasingly sophisticated guidewires - FIELDER XT-A, GAIA III, and ultimately achieving successful crossing with the MIRACLE 6 wire. Microcatheter support was maintained throughout the procedure to ensure precise navigation through the blocked artery.

The meticulous approach paid off. The completely blocked artery was successfully reopened, restoring normal blood flow (TIMI-3 flow) in the LAD. The patient's unstable angina stabilized immediately, symptoms improved significantly, and the final angiographic result demonstrated excellent restoration of cardiac blood supply.
